The check should be written out to: Baptist Mid-Missions Foundation, Incorporated

On a separate piece of paper,

please write my name and include it in the envelope (Shae Zabel).

3) Setup Automatic ACH Payments

Print and fill out a Declaration of Intention for Support including the needed ACH info and your gift will automatically be withdrawn from your account on a monthly basis.
